In your first visit to VIP please arrive a few minutes early to fill out your new patient paperwork. Please bring a valid form of ID, all of your insurance cards, and a pair of shorts. We have provided all of your new patient paperwork below if you choose to fill it out prior to your visit, please bring your completed paperwork with you.

At the start of your visit you can expect consultation with Dr. Hershman, a thorough medical history evaluation and a proposed treatment plan. At the conclusion of your first visit you will be scheduled for your ultrasound, if you don’t receive it on consultation day! We look forward to treating you like VIP!
